On-Line Athlete Application

The above application is for On-Line purchases only! The cost of an on-line purchase is $35.00. Payment by Credit Card is required at the time of purchase. 

Athlete Membership cards can still be purchased by mail for $30. If you wish to mail in your application go to Athlete Information,  print off an application and mail it to the office.  

Beginning in 2008-2009, USA Wrestling will require a  BACKGROUND CHECK in order to purchase a USA Wrestling Coach's Membership card. The cost of a background check is $17 plus the membership card fee. Due to the Background Check requirement, the ISWA will no longer be able to issue cards from the state office. All Coach's membership cards must be purchased on-line through the national office web site. Go to: www.themat.com
