ISWA Unsporting Behavior Form

Attention Officials: All tournament officials and staff are to use this unsporting behavior incidents form as needed to notify the ISWA office unsporting behavior. The ISWA office will then keep record of the incident. This should be completed as soon as the situation has been resolved or by the end of the tournament if possible. Please report within 24 hours of incident!
